2007/08 Live Music Now! Programs for Children - IN-SCHOOL REQUEST FORM

The Live Music Now! in-school demo/recitals consist of two 45-minute performances for two separate audiences at your school (i.e. K-3 first performance, 4-7 second performance). The concerts take place at 9:30 a.m. and 10:30 a.m. Schools should provide the following set up: one microphone, straight-back chairs, and a tuned piano (if needed).

You will be given 2 concert days (4 concerts). A “classical” performance will be scheduled first. Then on a separate date a “non-classical” performance will be presented.

Below, please provide information about your school. Next, check off if you have a piano and when it was last tuned. Finally, in order of preference indicate which 3 “classical” and “non-classical” groups you would most like to have. Also include 3 available dates for each genre. Priority will be given to early responders.
